I think the strength and conditioning coach was the one blamed for the players death. The university wanted to keep Durkin, but gave into pressure. As the HC he took the fall.

It was both of them. But what Kelly did was a lot worse than what Durkin did (or possibly didn't since as someone else posted it was most likely the S&C coach and he took the fall because he was head coach).
